Cat and Kitten Adoption Application Form

The first step in the process of adopting a cat/kitten(s) from OMC! (Only Maine Coons Rescue) is to complete this Adoption Application. We will use the information from the application to determine if the adoption is in the cat/kitten's best interest. This process ensures that you will find a cat/kitten(s) that is well suited to your lifestyle.

Please provide detailed information for all questions.

All of our cats come fully vetted for their age. Standard adoption vetting will included: testing for FeLV/FIV, 2 distemper shots, rabies shot ( will vary depending on age), ear mite check, fecal check, spay/neuter ( depending on cats age) Health Certificate if adopting out of state and required for transport purposes. OMC requires a prepaid spay/neuter agreement if the cat is not altered before adoption. Your adoption coordinator will explain the entire adoption process to you.

Please note that kittens under the age of 7 months must be adopted into a home with an exising cat or in pairs. Our adoption policy requires that an adopter have a cat already in the home, or that they adopt 2 cats from our group when choosing a kitten under the age of 7 months.

To qualify for the adoption process, you MUST meet the following criteria.

  • Be at least 25 years old and have a valid driver's license or state Identification Card stating your current address. (Please email your DL to info@omcrescue.org or fax it to 908-621-3215).

  • Have the knowledge and consent of a landlord, if relevant.

  • Be able and willing to spend the time and money necessary to provide medical treatment, proper nourishment, care, and training for cat/kitten(s).

  • You must be readily available for a home visit

  • Have an active vet account (If you have no pets, you will be required to setup a new account).
Please note: We reserve the right to refuse adoption to anyone. We will not adopt to persons who mislead or fail to provide ACCURATE AND COMPLETE information on this application.
