Cat and Kitten Adoption Application Form

The first step in the process of adopting a cat/kitten(s) from OMC! (Only Maine Coons Rescue) is to complete this Adoption Application. We will use the information from the application to determine if the adoption is in the cat/kitten's best interest. This process ensures that you will find a cat/kitten(s) that is well suited to your lifestyle.

All of our cats come fully vetted for their age. Standard adoption vetting will included: testing for FeLV/FIV, 2 distemper shots, rabies shot ( will vary depending on age), ear mite check, fecal check, spay/neuter ( depending on cats age) Health Certificate if adopting out of state and required for transport purposes. OMC requires a prepaid spay/neuter agreement if the cat is not altered before adoption. Your adoption coordinator will explain the entire adoption process to you.

To qualify for adoption, you must meet the following criteria.

  • Be at least 21 years old and have a valid driver's license or state Identification Card stating your current address.
  • Have the knowledge and consent of a landlord, if relevant.
  • Be able and willing to spend the time and money necessary to provide medical treatment, proper nourishment, care, and training for cat/kitten(s).

Please note: We reserve the right to refuse adoption to anyone. We will not adopt to persons who mislead or fail to provide accurate information on this application.
