Cat and Kitten Adoption Application Form
The first step in the process of adopting a cat/kitten(s) from OMC!
(Only Maine Coons Rescue) is to complete this Adoption Application.
We will use the information from the application to determine if the
adoption is in the cat/kitten's best interest. This process ensures
that you will find a cat/kitten(s) that is well suited to your lifestyle.
Please provide detailed information for all questions.
All of our cats come fully vetted for their age. Standard adoption
vetting will included: testing for FeLV/FIV, 2 distemper shots, rabies
shot ( will vary depending on age), ear mite check, fecal check, spay/neuter
( depending on cats age) Health Certificate if adopting out of state
and required for transport purposes. OMC requires a prepaid spay/neuter
agreement if the cat is not altered before adoption. Your adoption coordinator
will explain the entire adoption process to you.
Please note that kittens under the age of 7 months must be adopted
into a home with an existing cat or in pairs. Our adoption policy requires
that an adopter have a cat already in the home, or that they adopt 2
cats from our group when choosing a kitten under the age of 7 months.
OMC Rescue is pleased to announce our senior to senior program. If
you are 66 or older you qualify to adopt one of our senior cats, age
6 and up. If you are interested in participating in this program please
complete the application below. Once assigned you may speak to your

To qualify for the adoption process, you MUST
meet the following criteria.
- Be at least 21 years old and have a valid driver's license or
state Identification Card stating your current address. (Please
email a copy of your DL to email@example.com).
- Have the knowledge and consent of a landlord, if relevant.
- Be able and willing to spend the time and money necessary to
provide medical treatment, proper nourishment, care, and training
- You must be readily available for a home visit
- Have an active vet account (If you have no pets, you will
be required to setup a new account).
Please note: We reserve the right to refuse adoption to anyone. We will
not adopt to persons who mislead or fail to provide ACCURATE AND COMPLETE
information on this application.
